Use the Return of Scholarship/Fellowship Funding deposit form when depositing unused Scholarship.Fellowship funds. The completion of this deposit form is essential to the Office of Finance and Treasury to ensure proper tax treatment.
One form should be completed for each student deposit.
On the Return of Scholarship/Fellowship Funding deposit form enter the required student information.
Complete the ”Original Disbursement and Return Details” section. Include the funding type (i.e. Thesis, Fellowship, etc…) in the disbursement description. Enter the return details for the funds being deposited. Please note: No information is required in the tax withholding section.
For each check, enter the amount, the account code, department code, project grant, fund code and budget year under the ‘Credit’ section.
Photocopy the deposit slip and all corresponding deposit information. For check deposits, please keep a record of the following: issuer of the check, amount, contact information for the issuer, and check number. If a copy of the check(s) is made, it should be kept in a secure place and destroyed after 90 days.
Place the completed form and check(s) in an envelope and forward the deposit via interoffice mail to Cash Management, 701 Carnegie Center, Suite 432. If your deposit includes cash, please hand deliver to the Financial Service Center, 7 New South.
If forwarding checks for deposit via campus mail, assume the deposit will reach Cash Management two days after it is sent. You should see the deposit in your department account within seven business days of the time it was received by Cash Management.
