If your paycheck was lost or damaged, you may request a replacement check. The replacement check will look exactly like the one that was lost or damaged except for the issue date and check number. If you find a check you thought was lost, do not attempt to cash it.
Unless payroll discovers that the check has been cashed, you will receive a replacement check within 10 days of your request. If the check has been cashed, payroll will contact you for more information.
Complete a Stop Payment/Check Cancellation Form.
If you have the damaged check, attach it to the form.
Have your Department Head approve the form.
Fax or scan and email the form to payroll.
